Dr. Rafael Hernandez

Internal Medicine & Geriatrics | Value-Based Care Expert
Dr. Rafael Hernandez is originally from the Dominican Republic, where he attended medical school. Dr. Hernandez completed his residency in New York City at Woodhull Medical Center and his geriatric fellowship at NYU Langone Medical Center.
Early in residency training Dr. Hernandez found his passion for leadership and quality of care. He was the Committee of Interns and Residents (CIR) representative for his residency program and Vice President for the NYC chapter. In order to improve the quality of care of his patients, he became the Co-Founder and first Chairman of the House Staff Safety Council, focusing on patient safety and quality of care. He was also recognized as the Fellow of the Year by NYU professors and medical students.
In addition to Internal Medicine and Geriatrics, Dr. Hernandez (https://domuscaresolutions.com/category/blog/) is a recognized expert in value-based medicine and clinical leadership. He worked for the last five years at Chen Medical, where he was the National Director of Acute and Post-Acute Care.
Dr. Hernandez founded Domus Care Solutions (https://domuscaresolutions.com/home-based-primary-care-provider/) to fulfill his vision of enabling seniors to age in place by utilizing state-of-the-art, quality cost-effective home-based care with a special emphasis on the psycho-social needs of the patient.
He lives with his wife and two children. He enjoys spending his time off with his family, coaching his son’s baseball team, reading and taking golf lessons.
